The SERVICE PROVIDER understands that American Vision Health Plus (AVHP) is not an insurance company. It is a DISCOUNT driven membership program. The Service Provider understands that there are no fees to partake in the network.The network includes providers of: _______________________________________________________________
The Service Provider determines the discount of service(s) and/or product(s) offered. A NETWORK MEMBERSHIP DISCOUNT sheet is attached to facilitate selections. If at any time the Service Provider changes the fee or type of discount, the provider will submit changes to AVHP via fax, email or letter to amend this agreement. AVHP will reply with confirmation of updates to the Service Provider.
The Service Provider understands that these discounts are offered to members only. AVHP is not a payer and not responsible for any payments or reimbursements of such services provided to the members. All payments for services rendered are paid by the member presenting the AVHP discount card, on the day of service, based upon the Service Provider’s pre-established discount price(s).
The Service Provider also agrees to allow web link access to AVHP members from the membership web page. AVHP is permitted to publish information about the Service Provider for the purpose of promoting the products and services offered for AVHP members only. AVHP will not share the service providers’ customary fees and discount offers with any other institution, competitor, or other organization, contained in this agreement.

Since the patients of American Vision Health Plus pay at the end of the visit- The transaction is finalized right there and then and does not require additional docomentation. This is going to bring savings in materials, time, and resources to the provider.
